Adding A Form On Dreamweaver MX

Hi I need some help! I am making a site using Dreamweaver MX and whilst setting up a form for my customers to contact me. But its is asking me info on a testing server :confused: could you give me info on what to right in the following fields
server model
this site contains
access
FTP host
Host directory
URL prefix
if you could reply ASAP please if you need anymore info or anything email me at rory@getmegames.org

You can find all that info in your member's area at http://www.siteground.com . Basically, your details should be like:

FTP host = yourdomainname.com
Host Directory = public_html
URL prefix = empty

siteground has a place where you can make forms easily. Go to Fantastico located on the Cpanel and if you look towards the bottom on the left where all the links are listed, there is something called phpFormGenerator. All you do is make a form there after you download it then it gives you a url to where that form is located later so you can link to it.
see i have a form on my website. if you click on contact in my navigation bar you will see it.
